Families often find an urgent need for care when hospital discharge is reliant upon having the appropriate levels of support and care in place. We can react quickly, liaising with the discharge team and any other professionals involved in your care, allowing a prompt and safe return home. An at-home caregiver can help with recuperation, convalescence and rehabilitation, working alongside physical therapists and Occupational Therapists when patient health is often at its most fragile, helping to build strength and confidence, day by day. Our caregivers can also provide non-medical support for PEG, suppositories, and catheterizations, just to name a few.
Changing care agencies

From Care Home back to your own home
We support a lot of families who have tried a residential setting and discovered the pull of home was too much. Once you’ve made the decision, we can come and assess you both at the care home and your actual home, to help understand how we can ensure that your transition, both emotionally and physically is a smooth one.
